If Joe is 25th-Ed, all he has to do is communicate that whatever conditions existed that made him incapable to perform his duties, no longer exists. Then it becomes a fight between the Prez, and the VP plus Cabinet. If not resolved, the question then goes to Congress to decide.

As for the idiotic Jill is running things myth, no power of attorney would apply past his personal affairs, not his job. Some thought Eleanor Roosevelt was actually running the country long before FDR died, but that was mainly for public appearances. She had opinions on policy that she expressed, and had public opinion on her side, but was she actually in charge? I’ll let others debate that.
