looks to me like those tiles are 100mm x 100mm (4" x 4") t. extilerfag

extrapolating from there i get

a square opening of 300mm or 12"

almost impossible to 'fall' through an opening of that size IMO

plus it looks like there is an obstruction , on both sides of chute , and the hinge of the door itself , visible on the right side of the picture making the actual opening smaller still

i didn't even mention the height off the ground , dig for the height of the woman , or the probability that , that door doesn't open 100% of the way. i.e not 90 degrees from the wall , making the opening even smaller still. from my experience those chutes are designed to prevent someone entering and therefore falling to their death (or being incinerated , some old school ones lead to an incinerator).

the ones i'm familiar with stayed open , however;

if you look at the right side of the pic , there seems to be a hinge type object and on further inspection it could house a spring , enclosed in sheet metal for protection.

but complete speculation , as there would be many different manufacturers' , design and safety regulation changes over the years
