Like how they cited "news articles" to establish probable cause for their warrant. On multiple occasions, Page professed that "Candidate 2" (HRC) was trying to railroad him and that the evidence was made up. Judge signed the warrant anyway. Judges ignored A LOT of exculpatory evidence. Strzok's signature is redacted. Dates, redacted.
That's the list of individuals who are authorized to sign off on a FISA application. They have pre-printed shit so the undersigned can just sign it on his/her respective "line" without having to add their title. In other words, it's like an assembly line.

Not as far as I know. There's very little, if any opportunity for review of one of these orders. They rubber stamp them, clearly. I'm not even sure if they can be appealed as the target of a FISA application would not necessarily have Article III standing since the person could be foreign, never set foot on US soil. But an American citizen? I'm sure a creative attorney could establish Article III standing to challenge one of these orders.
All of these judges are appointed by the Chief Justice. Seven year, non-renewable terms. Congress can impeach them.
