• Secret Service didn’t visit Butler Memorial Hospital before the rally and didn't notify Hospital staff of the rally in a timely manner

• Secret Service agents didn’t follow the standard plan for emergencies.

• SS agents brought Trump through an unsecure Hospital entrance

• There was confusion on communication channels after the shooting

• After the shooting, EMS agents were still at the rally and weren't available to Hospital for support
