Steganography is one way (password/passphrase needed for decode).
Drops with prearranged info in a public space, radio, newspaper, TV, 8ch, are another way. "Numbers station" approach.
Any prearranged code can be used to send a message.
A code that's not prearranged though….public key cryptography can be used to privately negotiate a shared key…but that does not seem to be the case with this image.
There are probably an infinite number of ways a message can be hidden in plain sight, if the two parties agree on the meaning of the visual, the timing, the number of letters, a particular letter within the message, the checksum of the image, checksum of the message.. . . . . . . really endless.
