Imo q pointed to the fact that the blackhats wanted to push the narative that china hacked the sub and launched the missile (people found a related article after reverse image search of q's submarine photo.
As for the missile launch, i think q meant that there was a washington mole who ordered the launch and he was taken down secretly. The sub still couldve been hacked, thus fucking with its communication leaving only one chanel of communication straight to the washingtin mole:

This washingtin mole could lie to the captain of the sub that there is a certain threat and order him to shoot. The captain unable to reach others or get any outside info had to follow the order.
