From what I read, he trusts no one, so if they gave him meds, he'd see that trying to poison him, or control him, and just the thought would cause a mania, not good for all of his medical issues, it just enhances delusions at that point.
I've aided many people, with mental issues, and without any help, anyone understanding, lack of sleep just makes them worse, so it's a vicious circle of no sleep, causing hysteria/delusions, when sleep is needed most, but the paranoia won't allow it.
