In the year 2154, Earth is a polluted, overpopulated wasteland where the poor suffer from disease and poverty, while the wealthy elite live in luxury on Elysium, a pristine space station orbiting above the planet.
Elysium features advanced medical technology, including Med-Bays that can cure any illness or injury instantly.
The film follows Max Da Costa (Matt Damon), a former car thief and parolee working in a factory on Earth, who is exposed to a lethal dose of radiation from an accident at his workplace, giving him only five days to live.
Desperate to survive, Max seeks help from Spider (Wagner Moura), a smuggler who runs clandestine flights to Elysium, and agrees to steal valuable financial information from Armadyne Corp CEO John Carlyle (William Fichtner) in exchange for passage.
Recognizing Max's weakened state, Spider surgically implants Max with a powered exoskeleton, granting him superhuman strength, and an implant to store the stolen data.
Meanwhile, Elysium's Secretary of Defense, Jessica Delacourt (Jodie Foster), enforces strict anti-immigration laws by deploying a ruthless mercenary, M. Kruger (Sharlto Copley), to shoot down any unauthorized shuttles attempting to reach Elysium.
Delacourt, disapproving of her methods, is reprimanded by Elysium's President Patel (Faran Tahir) and dismissed from her position.
In retaliation, she bargains with Carlyle to create a program that can override Elysium's central computer and install her as president, which he encrypts within his own brain.
Max and his friend Julio (Diego Luna) intercept Carlyle's shuttle, successfully extract the program, but find it scrambled and unusable due to its encryption.
Delacourt sends Kruger to retrieve the data, leading to a violent confrontation in which Julio is killed and Max is severely injured.
Max retreats to the home of his childhood friend, Frey (Alice Braga), whose daughter Matilda (Emma Tremblay) suffers from leukemia.
After learning Max intends to go to Elysium, Frey pleads for him to take Matilda with him, but Max refuses.
With Max on the run, Delacourt orders an airspace lockdown over Los Angeles to prevent any flights to Elysium.
Spider, realizing the program can be used to make all Earth residents citizens of Elysium, attempts to launch a ship but is blocked by the lockdown.
Max is captured by Kruger, who takes Frey and Matilda hostage and forces Max to negotiate for their safety.
Max agrees to trade the data for a Med-Bay, and Kruger lifts the lockdown, allowing the group to travel to Elysium.
During the flight, a fight ensues, resulting in Kruger being severely disfigured by a grenade blast and the ship crashing on Elysium.
Max, Frey, and Matilda are arrested and taken to Delacourt, who plans to extract the data from Max's brain, knowing it will kill him.
Max escapes, frees Frey and Matilda, and reunites with Spider, who has secretly arrived on Elysium.
They head to the main computer core, where they are confronted by Kruger, who has been revived by a Med-Bay and now wears a superior exoskeleton suit, planning to seize control of Elysium himself.
In a final battle, Max disables Kruger's connection to his suit, and when Kruger attempts to detonate a grenade, Max throws him over a ledge to his death.
Spider connects Max to the computer core, but the data transfer will kill Max. Knowing this, Max says his goodbyes to Frey and activates the program.
As he dies, the Elysium computer reboots, registering every Earth resident as a citizen of Elysium.
This triggers the automatic dispatch of medical ships loaded with Med-Bays to Earth to heal the millions in need.
