Don't forget advertising -lots of cash and IMHO part of the pharma problem. "direct-to-consumer advertising is to directly influence the patient-physician dialogue in order to increase sales of a particular pharmaceutical drug" and permitted only in a few countries: USA, Brazil, HongKong & NZ (last unconfirmed)
