They were introduced as joke mascots for South Park Elementary School after the original cow mascot was protested by PETA, with Kyle Broflovski suggesting the Giant Douche and Eric Cartman proposing the Turd Sandwich.

The episode uses these characters to critique the political landscape of the 2004 election between George W. Bush and John Kerry, arguing that elections often force voters to choose between two undesirable options. Stan Marsh refuses to participate in the vote because he views both candidates as equally pointless, leading to his exile from South Park and ostracization by the town.

This concept was revisited in the 2016 Season 20 premiere, "Member Berries," where Herbert Garrison was cast as the Giant Douche (representing Donald Trump) and Hillary Clinton was depicted as the Turd Sandwich. The creators, Trey Parker and Matt Stone, returned to this theme to highlight the perceived lack of viable choices in that election cycle as well.
