It is supposed to make them look like "one of the people" but since they don't ever otherwise actually wear boots they have no idea how to do it.
Note that I assume yours was actually a rhetorical question but thought I would answer just in case.
Solution: Change the law so that insanity only delays trial, i.e. if a defendant is deemed incompetent to stand trial they are automatically committed to the relevant institution, then if at some time in the future they are deemed competent the trial begins. The Statue of limitations clock would have to be paused, etc. but there would be a lot fewer defendants going for the insanity defense, and they wouldn't just get back out on the street after a short stint in the loony bin. Oh and time in the loony bin doesnotcount as time served if they are convicted.
