The U.S. Chemical Safety and Hazard Investigation Board (CSB) released a preliminary report on the implosion Friday. It said a July 2025 inspection showed “significant thinning below the minimum safe thickness in the lower regions of the tank."
Two more inspections in October 2025 and February 2026 found the same results, according to the report.
