Yes that is the distinction.
If you buy your girl a pendant it is typically larger and it is a focal point.
A charm is smaller and it is added onto a larger overall piece.
The mixture of the two could be significant/symbolic.
Wearing something around your neck is pretty significant. It shows serious devotion. You wear your dog tags, cross, collar, etc… around your neck (one of your most vulnerable areas).
