don't doctors have to take periodic examinations in order to renew their medical license? Their university degree is for life, of course, but that in itself doesn't give them the right to operate on a patient.
An example is Steven Greer. He practiced medicine at a hospital in NC until the early/mid 90s. But he wouldn't be allowed in an ER now as he hasn't kept up with his exams.
