He is still alive. HE thought he would be able to escape by faking his death. Q owned the back-end of the death-faking system and set HIM up in the process leaving Q coincidence fingerprints ALL over it. In the future McCain will RETURN FROM THE DEAD to face trial for his crimes. It will be awesome. That is my guess.
