"Who" references a person. "What" would reference an item of sort.
Maybe he is aiming the AK47 at an American, or a photo of an American President/American political figure/etc.
Not sure how much she paid for this ad, but I am sick of seeing it on my twitter feed when I don't follow the crazy woman.
