128bits is indeed a block size for the older AES cypher. But it's a number that shows up quite frequently in computer systems. It's four 32bit integers. Heck, that's the address length chosen for IPv6.
We should save all the data. But it won't mean anything until we have context.
Personally, I think it's just a programming error: a hash table is part of a data structure somewhere and a bug is causing the wrong bytes to be sent into the wrong database fields.
