I have a couple of possible things to look at with the Ginsburg situation. First, the fall. Even at 86 Ginsburg is in relatively good health apparently. She is active. Falling can be a serious sign of blood pressure issues and/or heart issues. We know she has a past medical history of heart disease. Possible things with her age and history I would work up:

 

  1. Arrhythmia

  2. Orthostatic hypotension

  3. CAD/heart attack

 

That said, I believe there is a lot more to this. Ribs are moderately difficult to fracture without a serious trauma or underlying pathology that weakens the bone, i.e. osteoporosis, cancer. A fall causing this in an elderly woman would typically not yield a broken bone in the ribs but would rather be more likely in the hip or distal radius. This would make me very suspicious of a cancer–likely something similar to multiple myeloma–that is also concerning for the cervical lymphodenopathy we have seen in other pictures (the huge knot on her neck). It could also be a metastesis of her prior pancreatic cancer which would be much more ominous, and given her recent memory "lapses" and this fall could indicate brain involvement.

 

Of course, all of this would be conjecture. If it is related to her past cancer this is a very omnious sign. weeks to a few months.

Specific to this current event, 3 fractured ribs are no joke. Assuming there is no underlying tissue (punctured lung, liver laceration) she still is in a lot of danger. It is exceedingly painful to breathe deeply with 3 ribs broken. This causes you to breathe shallow and not cough up sputum. She is a a very high risk with her past medical history, immune status, age, and the time of year to develop pneumonia.
