This is the surreptitious introduction of error into data bases where as long as it goes unrecognized, error persists in memory, and the magnitude of the error compounds with distance from the calculation.
This is a serious attack.
Intentionally planted or unintentional Faulty Assumptions come to mind. It is good to revisit assumptions early and often to check the validity of their basis.
