What constitutes election fraud?

For the average voter, what do we look for that would signal us that we should alert someone?

I'm curious because when I voted on 11/6, the guy who was standing by the ballot box said that we had to put the ballots in the side of the machine (open slot that had no counter) instead of the top that sucks in the ballot and ups the counter.
