I don't know which resolution it uses, let's say its' 1 minute.
For each minute that passes you add the new visits and subtract the visits from 24h + 1 minute ago.

I'm not sure how the site actually works. I was trying to explain that it isn't a count of live users.
The math involved it's surely a bit more complex probably by considering returning users (same IP) to avoid counting them more than once.
Anyway given the exposure of Q we've seen I'd say it's totally believable
