Yes, there is always a possibility of a fatal reaction. But the chances of dying from a reaction to a vaccine are much, much less than the chances of dying from the disease if you were to catch it. Vaccines are generally good. But, I suspect, the problem comes when they hit an inexperienced immune system with too much of too many things at once. There are so many vaccines nowadays that doctors want to do too many in a single visit.
