Ok, medicalrecordanon here. I looked at that picture on a larger screen and without having access to that particular hospital’s database, I’m 99% sure this is real. The reason for all the different numbers on the far left, is every separate visit is a separate ‘code’ so to speak, for record keeping and insurance purposes. A patient’s MRN number (their identying code) may change from visit to visit..I cannot quite see if this is the case here though. You will also notice that some of the dates on the far right are blank, but that might be something as simple as a report not being yet completed/uploaded/finalized into the system. Take this with a grain of salt and yes, I could be wrong, but in my own personal experience, it looks legit.
