Perhaps all Christians in western Europe, but not all Christians worldwide.
The Original Church had five Patriarchs - Alexandria, Antioch, Constantinople, Jerusalem, and Rome. In 1054, Rome broke off their communion with the other four by “excommunicating” them, but they technically did not have the power to do this. More or less, Rome left and the rest of the patriarchs stayed where they were. Rome transformed into the Roman Catholic Church, while the others became known as the Eastern Orthodox Church.
This was the Great Schism. But, there were other churches as well. The Oriental Orthodox broke away from what would later become the Eastern Orthodox Church hundreds of years before this. In fact, the Coptic Orthodox Church was established by Saint Mark himself around 50 AD.
Therefore, the idea of all Christians being Catholics, that is, under the guidance of the Pope at Vatican City, is historically inaccurate. In fact, the Roman Catholic Church as we know it today was not formally established until it broke away from the East and took to governing itself with one supreme leader.
Not all Christians were Catholic before the Reformation.
