the idiom is very old. I recently heard it said on a 1981 situation comedy.
It always means the way I say
'thinking to the point of confusion, being stumped by a difficult situation, thinking too hard and being frustrated'.
it only has meant that.

Words and phrases do have multiple idiomatic meanings. You can't just make up a meaning and expect people to understand it.
