The spillway isn’t the main problem, it failed before and the hillside under it didn’t erode away much.
The dam itself has some seepage through the dirt it’s made of, that’s what can cause a failure but it’s been like that so idk
The main problem is if the Cabal wants a major ff they could trigger a big quake and liquify the dam
So they have an alibi

I just remember seeing videos of the dam last time and it was clearly seeping, there’s even trees or bushes growing along the seep lines. People were commenting about it.
As far as man made earthquakes, lots of people have made credible arguments for how that has been done, shouldn’t be hard to find but I’m phonefaging at work.
