On 4th down, you can do anything you want: kick, run, pass. But if you fail to get a 1st down, you will turn the ball over to your opponent.
In the parody video, they say it's the first play of the game and little Tommy certainly gets 10 yards to make it 1st down again, however, little Tommy fumbles the ball before he is down. Whichever team recovered the fumble would have offensive possession.
