That looks like significant neurological impairment. I remember when I saw that picture, I thought, "he's done, that's terrible, he's not even going to get to star in his role as the man to unite the Dems against POTUS."
Also notice the detached earlobe, that's the real Joe. No wonder they needed to bring in a ringer.
