If an attacker knows what parts of an encrypted message are in a stream cypher encrypted message then they can change those parts without actually needing know the key. There are ways of detecting if a message has been altered using hashes. And applications are supposed to check this. Thunderbird was loading external images (!!!) even if this check failed. THAT was the problem. If the NSA could break PGP directly they would have done so.
