On the bulkhead, near the window by POTUS, you can see the light cast in the shape of the window the picture was most likely taken from. Also in that window-shaped light cast on the bulkhead is the shape of the head of the picture-taker peeking in the window. The photographer, peering into the window must be well-known to POTUS and Ivanka because they seem at ease and just amused by the whole thing. The ghost head appears to be wearing hearing protection one would expect ground crews to wear. My guess is Scavino or staff got up there where ground crew were working(window washing or something- but where the photog was able to go. The photog may have captured (I think) the back of a ground-crew member’s very close-shaved, shiny head with ear protection headset visible. The ground-crewmember might not be wearing the headset with the connecting band across the top of the head, but maybe in a upside down or sideways fashion.
