I am linking the NTSB investigation of the 12Feb09 Q400 crash that killed some people of possible interest. So far, I have found nothing to dispute the conclusions (see page 165 of the pdf). FWIW, I was involved with multiple USAF AFSAS safety investigations over the course of a dozen years.

Odd that NTSB chose "probable." We knew better than to use that word or "likely" with a Board President.
