After the droplet dries up, yes, the resulting particle is too small for a normal surgical mask. But they are perfectly capable of catching a still-wet sprayed droplet from the person wearing it. That's why they're worn in operating rooms. So it makes sense for a caregiver to wear a mask just in case they unknowingly have it and wouldn't want to spread it to the elderly people around them.
