Trump wouldn't need to give any kind of permission for an off-label use of a drug. Most prescriptions (billed to insurance) do not require a diagnosis code to go along with them.

A doctor can write a prescription for anything and pharmacies can fill it without knowing, reporting, or documenting the diagnosis. The real question is how it will be handled by insurance companies if the RXs are being written for quantities/doses that are outside of the way it's normally used, but all of that can be bypassed by paying out of pocket and even so, so many companies are complying with the effort that I doubt there will be much resistance. I'm already seeing available overrides just for prescription refills that are too soon.
That is definitely a consideration.
Maybe this is part of the reason he put so much focus on the economy. We needed to be ready to absorb the blow.
