FFS, if you had diabetes and got in a car accident and died, the cause of death would be the car accident, not the underlying condition of diabetes. If you had gout and caught malaria and died, you'd be marked down as a malaria death, not a gout death. It's what puts you over the top and into the grave that gets marked as the cause of death.
