Reposting the NTSB report. I read it and couldn't find any conclusions to really disagree with. I was involved with multiple USAF safety investigations in my career.

Except for one technical aspect that I don't remember being addressed. One engine's throttle lever was at full power and the other was mid-range. I'm going to dive back in and get my hands wet again.
