How is this happening? Well we have frequently discussed the ACE2 receptor in the lungs but we have to also remember that there are ACE2 receptors on endothelial cells throughout the body. It is believed it uses these ACE2 receptors on endothelial cells to enter these cells in areas other than the lungs.

On autopsy of COVID-19 we are seeing the small vessels full of clots in the lungs, bowel, liver and kidneys. In a recent French study comparing 150 patients with COVID-19 patients with ARDS in the ICU to 145 patients who had ARDS without COVID-19 found significantly higher rates of blood clotting in the COVID-19 patients.

This suggests that SARS-CoV-2 infection facilitates the induction of endotheliitis in multiple organs due to both viral involvement and the effects of the inflammatory response commonly called cytokine storm.

So we are now learning that clots are a big part of the COVID-19 disease. This is likely the etiology of strokes. So doctors are suggesting that drugs that support the endothelium like statins and ACE inhibitors might help protect one from this. On admission doctors are putting people immediately on anticoagulants to try to combat this. In addition, treating the inflammatory response as soon as possible to hopefully prevent the cytokine storm.

Since the virus is reportedly mutating will this affect the development of the vaccine?

Now this is actually a better question for a virologist or microbiologist but since multiple people have asked. Here is my take on it. A study out of China reports that the virus has mutated up to 30 times. While the SARS-COV2 virus has likely mutated some, historically the coronavirus is a slowly mutating virus. Remember that immunity or the vaccine work by essentially binding to the antigen which is the protein on the virus. On the SARS-COV2 you have the M protein and the (spike) S protein. So it will take a significant enough mutation to change the M or S proteins enough that the antibody will not recognize it to stop binding to it. That is usually a significant mutation so I am not currently worried about this.

Zinc any amount is probably good but take what you can tolerate without making you feel bad up to 40mg. You do not want to be Zinc deficient as it is how many drugs like HCQ work.

Quercetin from 250mg up to 1g twice per day Remember this is a Zinc ionophore.

Green Tea either drink some or 1 pill per day alternate zinc ionophore

N-acetylcysteine (NAC) - 600mg twice per day shown to decrease severity of influenza, improve lung functions and help with mental disorders.

Melatonin 1mg to 10mg at night. Take what you can tolerate and what helps you sleep without making you feel drugged or tired in the morning.

Vitamin C 250mg to 500mg twice per day. Effects debatable in oral form but IV did help in China. But is proven to strengthen immune system

Vitamin D3 Take this for sure if you are deficient and I now recommend taking it as long as your values are not too high.

Sleep greater than 8 hours per night
