Just like any other medication, there are relative and absolute contraindications to using it. It is found to cause a prolonged QT (electrical heart rhythm) disturbance which can lead to a fatal arrhythmia; from what I've seen this is in long term and/or high dose usage of HCQ. Certain medical conditions or medications taken would prohibit someone from being prescribed this, or certain "extra" precautions put in place, such as remote heart monitoring. The key is this is no different than any other medication with indications, contraindications and precautions.
