Are these accidents a combination of bad equipment and lack of "old school" backup systems? Today we are so reliant on electronics that the skills and systems that just work are no there.
Think of the scene from Top Gun (not saying it is historically accurate, but it hammers home a point). Kills in Korea were 12 to 1 because our guys knew how to fly and had to get close for a kill. Vietnam it was 4 to 1 because everyone became enamored with a missile and being able to kill from a far.
Everyone wants more tech to do everything for them; drive, fly cook, clean. When we forget how to do things ourselves we become mindless zombies.
