The civilizations the heads represent are in the context to the Saints of God, whether before the flood or after, those among the Jews, and later those among the Gentiles.
The first head was pre-flood Sumer and pre-flood Egypt, both founded by Cain. Post-flood Sumer and post-flood Egypt were a continuation of what Cain had founded.
The second head was Assyria, arisen from post-flood Sumer, being refounded by Cush (Kush) the grandfather of Nimrod the "mighty hunter before the Lord".
The third head was Chaldea (Babylon).
The fourth head was Medo-Persia, including Haman the Agagite.
The fifth head was the Helenistic (or Macedonian) Empire (called "Greece").
The sixth head (contemporaneous with John the Apostle) was the ancient Roman Empire, including after it had moved its seat to Constantinople. It was at this time Jesus Christ remitted the sins of His sheep, defeating the Dragon in Heaven so he could no longer accuse the Saints before the throne of God. Therefore, the head is said to be "wounded", since of course this head also was controlled by the Dragon. This "wound" is referring to the removal of the power the Dragon had against the Saints in accusing them as promised in Genesis 3:15. He could no longer accuse the Saints of sin before God's throne, no longer control them through sin. Thus, his head was "wounded" in that he no longer ruled over them. He had been "bound" by the Gospel, however not bound from controlling the Beast as the Roman Empire continued. It was during the time of this head, the Great Falling Away occurred later also.
Since the sixth head had been wounded by the GOSPEL, having removed from the Dragon the power to accuse the Saints in Heaven, and since the apostate church re-established a priesthood which perpetuated that sin sacrifice of Christ, DENYING THE GOSPELin essence, saying sin is still being imputed against the people of God so that they need to constantly resort to the eating of that perpetual "sin sacrifice"then the "wound", that is, the ability of the Dragon to accuse the "saints" of sin, is revived, but not before God's throne in Heaven. The apostasy is what revived that wounded head. It was revived in the world in the guise of service towards God, and later when the seventh head came to rule, the authority of the seventh headthe Antichrist, the Papacyfought against the Saints defending the Gospel against this apostasy. They were accused of being heretics and were overcome, the wars waged against them during the time of the seventh head done in the name of "the cross" (crusades).
The "woman" riding this beast during the time of the seventh head was the APOSTATE CHURCH. She represents the priesthood of the APOSTASY who controlled the beast when the seventh head had awakened. In all of the pagan phases, the emperor was always both its king of kings (emperor) and its high priest. In the phase of the sixth head this was ended because Emperor Phocas in the east relinquished his ancient Roman title of "Pontifex Maximus" (high priest), giving it over to the bishop of bishops, an office created by Phocas's forebear, Justinian I. From that point, the emperor was no longer considered both the king and high priest as pagans had donejust its king. The Papacy was made the high priest. The high priest of the apostate church was actually the power behind the throne of the seventh head (the Holy Roman Empire), so the "harlot" is said to "ride" the beastthat is, to control (rein) its movements through the high priest, the False Prophet, and his priesthood.
The seventh head was the Holy Roman Empire (as we have come to call it) whose emperor was controlled by the little horn called the "Papacy"–the False Prophet, the Man of Sin.
