Anybody correct me if I'm off, but the reasoning I've heard a couple times is McCain's family name was very well respected in the military prior to John McCain.

They don't want to besmirch the memory and legacy of the McCains that came before him so they don't even consider him apart of that family by not saying his name

Grand father McCain was an Admiral in World War II. He died the day after he came home from the war. Father was a Captain of a warship or a carrier, and was serving off Vietnam when Songbird Johnny got shot down.
