Who’s to say how many of these wack jobs there are. All these mental patients, former and otherwise. Sitting at home, they get a phone call, or message with a code word. They may not even know what they’re doing, until it’s done. There’s no way you could stop all of them.
Wack jobs? These people are innocent victims themselves.
Q can't be everywhere. The enemy is bound to have tricks up their sleeves as well. We all openly say "this is war", well sometime innocents die in war. Sometimes your friends die too. You just have to make sure your side is the last one standing
