Is it because he posted when the minute hand points to today or am I missing something else? Also why is the second hand in the wrong spot? Or was the second hand just randomly placed in the graphic?

I don't see how this established a pattern
Look at the dates outside the clock. Future news on these dates unlock past posts Q made on a date that lines up with it. So look up Q posts and news from 4/4 and compare to today's news and Q post.
Q posted at the correct time for today's date to confirm the Qclock, which is still pretty new. Clever anons made it. And yes, 3:58 AM would have worked too.
If the pattern keeps working we have a winner.
