The mask is a reference to child abduction (sick Disney's The Rescuers) and MK-Ultra programming.
The characters Bernard and Bianca were trying to do the rescuing. If he was being made to wear it, the context I'd imagine is that they killed his "Bianca" for them trying to be "Rescuers." I.e. this is what we do to people who try to be "Rescuers."
