They randomly trigger. There's no way to stack the odds. If you don't have a NM proc after 3 raids, your 4th one will have a 100% proc rate. Maniac raids always have a 100% proc rate.
NM can proc from all of the difficulties, but only extreme has the 3 cutoff. Either way, extreme gives the best repeatable drops so you should do that if you can.

There are 2 6% fests. Legfest is at the end of the month and grandefest is midmonth. Though it seems they will move grandefest to the end of the month for a 2 part legfest like with new years or summer
