The silicone bridge is eroding through the thin layered tissue due to a lack of blood flow . As we age our skin becomes thinner. This type of implant only allows blood supply from the skin it's self. In this case a bone graft should have been used (rib) allowing new vascular formation from below not just within the thin surface layer of dermis stretched over the silicone implant. Ulceration is the result.
