Here are the two questions we tried to ask in our Google Surveys survey:

1. Has anyone in your household (including yourself):

A. Had Covid

B. Is now unable to work because of a COVID infection

C. Died from COVID

1. Has anyone in your household (including yourself):

A. Had the COVID vaccine

B. Had a diagnosis of myocarditis after getting the COVID vaccine

C. Died from the COVID vaccine
