There was an article a while back that said 100% of deaths were from 5% of the lots. Not sure if those numbers have changed over time, or even if that report was credible, but I do think placebos, or just a typical, mostly harmless, flu shot was somehow used to replace the [deadly] formula that was intended to be widely distributed.
