The ATC tapes clear the controller. There are two versions available. One with PAT25 audio and one without. The controller hears and responds to a conflict alert between PAT25 and the aircraft on approach.

Listen to/watch this exchange between DCA ATC and PAT25. ATC was not in the wrong. PAT25 repeatedly requests visual separation. The burden is then on PAT25 to maintain visual separation. The ATC double checks with PAT25 after receiving a conflict alert.
The only apparent anomalies are PAT25 reporting 200 -300 -> 400 feet (collision) altitude and the failure of PAT25 to maintain visual separation. Planefags please comment.
