I think the glaring reason there is huge amount of drug and alcohol abuse in Hollywood is not because they can't handle fame. It's because they can't handle what horrible act they did in order to get famous.
They can never forget. They can't confess. And, some can't live with it. Only option is to split personalities or abuse drugs
