So the FBI gave outside contractors access to FISA database and once their work was done, they just "forgot" to remove it. When adm Rodgers found out about this, Clapper and Carter tried to get him fired but failed.

The move to fire Rogers, which failed, originated sometime in mid-to-late October 2016—exactly when Rogers was preparing to present his findings to the FISA court."
